Community Development Services
Community Development Services serves as an alternative to traditional services and offers community –based experiences to small groups with staff supervision.
- Individualized scheduling based on the Person Centered Plan (PCP)
- Build relationships with others in the community
- Learn, practice and apply skills that promote independence and inclusion in the community
- Volunteerism
- Skills training
- Transportation support
Case Management:
- Oversees service delivery
- Communicates with the individual’s family, advocates, and stakeholders as needed
- Attend individuals’ annual PCP (Person Centered Plan)
- Works with direct staff to support the individual with achieving outcomes and goals, as developed by the individual and team
